Got some owning up to do with regards to the rules...

Firearm combat isn't opposed. Therefore any penalties are applied to the attackers roll. So, for example when diving for cover, the penalty dice is applied to the attackers roll. This really does make more sense now of how to apply the bonus/penalty die.

With regards to bonus or penalty dice however, this doesn't have to be just one. For example, if the attacker was making multiple shots and the target was diving for cover, he would receive 2 penalty dice to his first attack, then one each for the remainder. If the attacker happened to be aiming, a bonus die, this would net off one of the penalty die meaning only one is rolled for the first attack.

This gives dome indication on how bonus/penalty die stack.

Also, if diving for cover, you have to forfeit you next attack.
